Yes, all new patients must complete intake forms prior to their first appointment. Completing these forms in advance helps us provide you with the best possible care.

We work to address the root causes of anxiety and panic disorders and do not routinely prescribe Xanax. It is only considered in select cases when absolutely necessary.

No, we do not prescribe Suboxone.

Yes. All patients are required to submit a drug screening before receiving stimulants, benzodiazepines, or other controlled substances. This is a safety measure to prevent harmful drug interactions and ensure your well-being.

Co-pays are due at the time of your appointment.

Yes. Appointments that are canceled less than 24 hours in advance or missed without notice will incur a $150 fee.

Yes, if your insurance denies the claim, you are responsible for the self-pay rate. We do offer flexible payment plans to help make your care more affordable.

If you have an outstanding balance, our office will send up to three payment notices. If payment or a payment plan is not arranged after these notices, we may not be able to schedule future appointments.

Prescription refills can take up to 48 hours to be processed. Please plan ahead to avoid running out of your medication.

No. Our policy requires patients to be under our care for at least three months before we can complete any paperwork, such as forms for work or disability.

Yes, there is a $25 fee for each document or letter requested.
