YOUR VISIT CHECKLIST
What to have ready.
- Photo ID and insurance card, if applicable
- A list of current medications and supplements
- Relevant records you already have, including discharge instructions if applicable
- Questions and goals you would like to discuss
Ask the office how to share requested records and return forms. Keep medical information out of ordinary email and the appointment request form.
THE FIRST CONVERSATION
You do not need to have every answer.
An initial psychiatric visit commonly includes a conversation about your concerns, relevant health history, current medications, and goals. Your provider will discuss an assessment and possible next steps with you.
Bring your questions. Decisions about treatment and follow-up depend on your individual assessment.

Questions worth asking.
How long is the first appointment?
Ask the office for the length of your specific appointment and whether to arrive early for paperwork. Visit length depends on the appointment type and provider.
What will my visit cost?
Costs depend on your coverage, provider, and service. Ask about copays, deductibles, authorization requirements, and current self-pay fees before your visit. Our insurance guide links to plan information and member support.
When will I hear back after requesting a visit?
The scheduling team reviews requests and contacts you about availability. A submitted request does not reserve a time. Please call for time-sensitive follow-up or to check on a request; online requests are not monitored continuously.
What if I need to change an existing appointment?
Call the office handling your appointment to make a change. Avoid submitting a new appointment request for a visit that is already scheduled.
Do I need to send medical records before I request?
No records are needed in the public appointment request. Share scheduling details only. The office will explain which records or forms are needed and how to send them through an approved method.
