FROM DISCHARGE TO YOUR NEXT VISIT

A clear way forward.

Your appointment is scheduled when the clinic confirms it with you.

01

Call for follow-up

Tell the scheduling team you were recently discharged from an ER or hospital and need psychiatric follow-up. Let them know if your discharge plan recommends a particular timeframe.

02

Find the right visit

Discuss the patient’s age, care needs, preferred office or telehealth, and insurance. The team will review whether outpatient care at Crescent is appropriate and confirm the available date and time.

03

Prepare to continue care

Have your discharge instructions, current medication list, and insurance information ready. Ask the office how to share requested records through an approved method.

CARE THAT CONTINUES

Make room for
your next chapter.

Follow-up is a chance to discuss how you are doing, review your medications with a psychiatric provider, and plan ongoing care. Recommendations depend on your individual assessment.

Continue following your hospital’s discharge instructions. If your discharge team recommends a different service or a higher level of care, follow that plan and contact them with questions.

TELEHEALTH

Connect from Maryland

Ask about a remote follow-up while you are physically in Maryland. Telehealth depends on clinical appropriateness, provider availability, and your coverage.

SUPPORT AFTER YOUR HOSPITAL STAY

Follow-up close to home.

Crescent welcomes inquiries from people discharged from UM Upper Chesapeake Medical Center Bel Air, UM Upper Chesapeake Medical Center Aberdeen, and hospitals or emergency departments in Baltimore and elsewhere in Maryland.

Our offices are in Bel Air and Conowingo. Eligible patients elsewhere in Maryland can ask about telehealth. Crescent is an independent clinic; naming these hospitals does not indicate an affiliation, partnership, or endorsement.

FOR DISCHARGE PLANNERS, SOCIAL WORKERS & CARE TEAMS

A clear handoff
to outpatient care.

Call either office and ask for hospital-discharge follow-up. Coordinate with the patient and follow your organization’s authorization and records-sharing procedures.

Confirm before discharge

  • The patient’s age, clinical needs, and whether Crescent is an appropriate next step.
  • The actual appointment time, provider, visit format, and location.
  • Specific insurance plan participation and any referral requirements.
  • The correct recipient and approved method for transferring requested records.

Do not send discharge summaries or other medical records through ordinary email or the public appointment form. Call to verify the recipient and fax or other approved transfer method first.

BEFORE YOU REACH OUT

A few helpful
answers.

Your first-visit guide
Can I be seen today or the next business day?

Crescent offers same-day or next-business-day outpatient psychiatric follow-up after hospital or ER discharge. Call the clinic to discuss care needs and confirm the actual date, time, provider, and visit format. A website request does not reserve a slot or guarantee a same-day response.

Does this include weekends or requests made after hours?

Call the office to confirm the next available appointment and what “next business day” means for your request. Listed office hours do not guarantee an open appointment. Online requests are not monitored continuously.

Can my visit be by telehealth?

Yes, telehealth is an option for eligible patients physically in Maryland when appropriate for their care. The clinic will confirm coverage, provider availability, and joining instructions. In-person visits are available in Bel Air and Conowingo.

Do you accept my insurance?

Crescent accepts Medicare, Maryland Medicaid, and the plans on our insurance page. Participation and benefits depend on your specific plan, provider, and service. Confirm these with the clinic and your insurer before your appointment.

What should I have ready?

Have your discharge instructions, current medication list, insurance details, and any follow-up timeframe recommended by the hospital. Bring your questions, too. Ask how to transfer any records the provider needs; do not upload them into the appointment request.

Is this emergency or walk-in care?

This is scheduled outpatient psychiatric care. It does not replace emergency care or a hospital-recommended level of care. Call 911 for immediate danger or a medical emergency. Call or text 988 for mental health crisis support.
